Consent

of Member or Parent / Carer

• I understand that the club will comply with its Child Protection Policy and Procedures including changing rooms, anti-bullying, photography or video recording policies, and DBS (Disclosure and Barring Service) requirements. The Club Child Protection Policy is available for inspection at the club, and/or may be emailed to me upon request.
• I agree members may be photographed or filmed and that the club may use photographs and video recordings for educational or club promotional purposes (including social media), and for crime prevention and child protection purposes. I will inform the club by email/message/letter if I do not want the member photographed/videoed for any of these purposes.

All Can-Do Club workers are DBS checked and each one has a ‘Safeguarding in Sport’ certificate. Can-Do Sports and Drama Club Ltd is independently run by qualified experienced professionals from Specialist Schools and the NHS, who work daily with young people with additional needs. We do not use any volunteers in the sessions. The Club carries full Public Liability and worker insurance. 

Sports and drama play a major role in promoting inclusion of all groups in society, but inequalities have traditionally existed within sport - particularly in relation to gender, race and disability. Can-Do Sports and Drama Club is committed to promoting and developing sports and drama equity, which is about fairness in sport, equality of access, recognising inequalities and taking steps to address them.

Membership is free, and granted once this form is completed and accepted.

It's a busy two-hour session, so please arrive fed and hydrated. There is a Changing Places room for members, where personal care may be done before the session starts (please arrive early – we start promptly with the ‘Welcome Arch’). There is also a mid-way 10 minute break in each 2hr session where meds and liquids may be given by parents/carers if needed. Can-Do Club provides free tea, coffee, cold drinks and biscuits to parents and carers in the Can-Do Café area. You can cancel up to 48hrs before any session and receive a full refund.

Declaration

• I (member, or parent/carer) have read the information contained on this form and consent to (me/them) taking part in the club’s activity sessions, and I understand and agree that participation in sports and drama sessions is entirely at (my/their) own risk.
• I have considered the nature of such sessions (and have discussed this with them).
• As a parent/carer I understand that I will need to stay within five minutes distance from the venue in the event I am needed urgently (or for personal care).
• As a parent/carer I understand in the event of injury or illness all reasonable steps will be taken to contact me, and to deal with that injury or illness appropriately.
• I confirm that (I/they) do not have any medical disability or medical condition (not disclosed in the previous section) that could affect their ability to participate safely in sports and drama sessions.
